Important notice to all applicants

Eligibility Requirements
- Be a graduating senior,
- Be a resident of Walpole,
- Have participated in the Walpole Youth Soccer program, and
- Be continuing her/his education after high school.
Transcript Instructions
In order to have all the information for your application, please be sure that you request that your high school sends a copy of your transcript grades to the Scholarship Committee.
The transcript and the application should be sent before the June 10th deadline.

Conditions for Application
In consideration of the receipt and evaluation of this application, the undersigned understand and agree to the following conditions:
- All decisions by the WYSA and its agents will be final and binding. Accordingly, the applicants release WYSA, its officers and agents from any claims or liability in connection with the evaluation of the application.
- Applicants who become finalists as determined by the scholarship committee hereby agree that school authorities shall have the right to release to the committee information relating to class rank and any other information reasonably necessary to verify the application.
- The scholarship will be paid to the school, college or university on behalf of the scholarship recipient. Such payment will be made upon receipt of satisfactory evidence that the student has been accepted and has enrolled.
